What should I bring to my appointment?
*Please print forms and fill out to bring to your appointment.
*Insurance Cards
*If you are unable to print the New Patient Packet, please bring a list of medications and emergency/family contact names and numbers to the appointment.
*You must hand carry your X-rays and/or MRI films to the appointment. Do not depend on your doctor’s office or the hospital to send them. Please call the clinic or hospital where your X-rays were taken to arrange to pick them up prior to your appointment.

What is your prescription and refill policy?
Prescriptions and refills are only issued during our regular office hours before 4:00pm. Pain medications are not refilled after hours or weekends. When you call for refills, please have your medicine bottle available so we can obtain accurate information from you. All prescriptions have a 24 hour turn around period; please call before running out of your medication.

Our main office is in Asheville, NC. We also have five satellite offices in Brevard, Columbus, Marion, Rutherfordton, and Sylva, NC. For directions to these locations click here.

What is Carolina Spine & Neurosurgery Center’s payment policy?
Payment is due at the time services are rendered unless prior arrangements have been made. Co-pays are collected at check-in.
We accept Cash, Check, Debit Card, Mastercard, Visa, Discover, American Express, and CareCredit.

How do I get disability/FMLA forms completed?
You may drop your Disability/FMLA forms off at the MRI front desk or mail them into our office. All forms will need to be signed by the patient giving us authorization to release information regarding your medical condition. Forms are processed/completed within 4-6 days of receipt by our office. There is a $25.00 charge per Disability form and a $5.00 charge per FMLA form. What is HIPAA?
The Health Insurance Portability and Accountability Act (HIPAA) was enacted by the U.S. Congress in 1996. According to the Centers for Medicare and Medicaid Services' (CMS) website, Title I of HIPAA protects health insurance coverage for workers and their families when they change or lose their jobs. Title II of HIPAA, the Administrative Simplification provisions, requires the establishment of national standards for electronic health care transactions and national identifiers for providers, health insurance plans, and employers. The AS provisions also address the security and privacy of health data. The standards are meant to improve the efficiency and effectiveness of the nation's health care system by encouraging the widespread use of electronic data interchange in health care.
